The Director of Nursing was asked, "who is responsible for reporting all corruption allegations?" The Managing director replied, "she was and the Ambassador." The Director confirmed that they were enlightened of the incident involving both residents. The Director stated that one resident had wandered into another resident'south room who "was hollering for assist. Before the staff got to [the hollering resident, the assaulting resident] hit that resident in the oral fissure."

The surveyors asked the Director, "when was this was reported to the State?" The Director replied, "it was not reported to the State." The surveyors asked, "why it was not reported to the State." The Director stated, "nosotros did not see that as abuse."

The Director of Nursing indicated that the type of incidents reported to the state included "any abuse, neglect, or fractures of unknown origin." The Director also indicated that these incidents should exist reported: "within twenty-four hours."
